PA State police identified Middletown, Bucks County resident Walter Bereza, 86, as the one fatally injured in the July 14, 2011 accident on the PA Turnpike in Bensalem. Unfortunately, Bereza who was not wearing his seat belt while driving his pickup truck, lost control when a rear tire blew out. After fishtailing and hitting the center barrier, the truck began spinning and flipped over landing on its roof while Bereza was partially ejected from it. A Bucks County Deputy Coroner pronounced him dead on the scene of this tragic accident.
